Getting tested is easy, free and totally confidential. Just contact Team Chlamydia or one of our screening centres listed below and they will arrange everything. There is no need to be embarrassed - we are here especially to make testing and treatment easier.

If you are aged between 16 and 24 and live in the Central and Eastern Cheshire area, the good news is that free postal test kits are now available!

To request a kit, please call Team Chlamydia on 01625 663434 Monday to Friday 9 am to 4.15 pm (if you ring out of office hours please leave your name and contact number on our answerphone and we will get back to you as soon as we can) or use our secure online request form.

Please note that the test kit will be sent to you in a plain envelope and will include full instructions.

Confidentiality

Your name, address and details will be held securely by Team Chlamydia and treated as highly confidential information, used solely by the National Chlamydia Screening Programme and the Department of Health.  We will not pass this information on to anyone else.  You will only be contacted with your result by the method you choose (eg, text, phone call or letter).

Important:  The completed form you send with your test must include the date you do the test, your name, date of birth, postcode, sex, ethnicity and most importantly your contact details (mobile number, landline or address) so we can let you know your result.  Failure to complete the form could result in the test not being processed.
